We have a dedicated team working daily to visit patients with acute medical concerns. The team is made up of GPs, paramedics and advanced nurse practitioners.
Visits are booked by your own GP surgery who will have spoken to you first, identified you require a home visit and asked SPCL to come and assess you through our visiting service.
Not all visits are suitable for our team which is why you come through your own primary care team first. For example some of you will have long standing medical conditions, which may have been managed for years by a team who know you and everything you have previously tried to help this condition. If this is you, you are best managed by the team who know you, continuity of care is so important.
However if you potentially have an infection, have developed new pains or symptoms, we have a team ready to visit you.
Over the pandemic, SPCL saw all cases of Covid and suspected Covid requiring a home visit across the SO14-19 postcodes. It may be you are reading this and you have already met a number of clinicians on our visiting team.
